{"id":1464,"date":"2021-01-06T08:23:02","date_gmt":"2021-01-06T13:23:02","guid":{"rendered":"https:\/\/multi.nycnewsnetwork.com\/nycpolitics\/2021\/01\/06\/heres-the-full-list-of-nyc-covid-19-vaccine-sites-groups-that-are-eligible-and-appointment-information\/"},"modified":"2021-01-06T08:23:02","modified_gmt":"2021-01-06T13:23:02","slug":"heres-the-full-list-of-nyc-covid-19-vaccine-sites-groups-that-are-eligible-and-appointment-information","status":"publish","type":"post","link":"https:\/\/nycnewsnetwork.com\/nycpolitics\/heres-the-full-list-of-nyc-covid-19-vaccine-sites-groups-that-are-eligible-and-appointment-information\/","title":{"rendered":"Here&#8217;s The Full List of NYC COVID 19 Vaccine Sites, Groups That Are Eligible And Appointment Information"},"content":{"rendered":"<p>As of January 4, 2021, eligible New Yorkers can get the COVID-19 vaccine at sites located throughout the city.<\/p>\n<p>The following groups are now eligible for vaccine:<\/p>\n<p><strong>Health Care Workers<\/strong><\/p>\n<ul>\n<li>High-risk health care workers in hospitals and Federally Qualified Health Centers (FQHCs), including NYS Office of Mental Health (OMH) psychiatric centers<\/li>\n<li>High-risk health care workers who provide direct in-person care in outpatient\/ambulatory settings and other staff in these settings who have direct contact with patients (such as reception staff); this group includes workers in private or hospital-affiliated medical practices, specialty medical practices, public health clinics, dental practices and diagnostic and treatment centers; phlebotomists; dialysis workers; occupational therapists; physical therapists; speech therapists; behavioral health workers; and student health workers<\/li>\n<li>Emergency medical services (EMS) personnel<\/li>\n<li>Urgent care providers<\/li>\n<li>Health care workers administering COVID-19 vaccines<\/li>\n<li>Health care workers in COVID-19 testing sites<\/li>\n<li>Home care workers and aides, hospice workers, personal care aides, and consumer-directed personal care workers.<\/li>\n<\/ul>\n<p><strong>Residents and Staff in Certain Group Living Facilities<\/strong><\/p>\n<ul>\n<li>Long-term care facilities, including nursing homes, skilled nursing facilities, adult care facilities<\/li>\n<li>Long-term group living facilities run by NYS OMH, Office People with Developmental Disabilities, and Office of Addiction Services and Supports<\/li>\n<\/ul>\n<p><strong>Other Eligible Groups<\/strong><\/p>\n<ul>\n<li>Medical examiners and coroners<\/li>\n<li>Funeral workers who have direct contact with infectious material and bodily fluids<\/li>\n<li>Frontline, high-risk public health workers who have direct contact with patients or handle COVID-19 specimens or vaccinations<\/li>\n<\/ul>\n<p>More groups will become eligible in the coming weeks.<\/p>\n<ul>\n<li><a href=\"https:\/\/www1.nyc.gov\/site\/doh\/covid\/covid-19-vaccine-eligibility.page\">See the Planned COVID-19 Vaccine Distribution List in New York City<\/a><\/li>\n<\/ul>\n<h2>Where to Get Vaccinated<\/h2>\n<p>All of the below vaccination sites require you to make an appointment first. If you are currently not eligible for vaccination, do not sign up for an appointment at this time.<\/p>\n<p>Contact your employer to receive your vaccination if you work at or are affiliated with:<\/p>\n<ul>\n<li>A hospital network<\/li>\n<li>An urgent care center<\/li>\n<li>Federally Qualified Health Centers<\/li>\n<li>A congregate setting associated with the New York State (NYS) Offices for People With Developmental Disabilities, Mental Health, and Addiction Services and Supports<\/li>\n<\/ul>\n<p>For all other eligible groups, including eligible health care workers not affiliated with a hospital and eligible funeral workers, first check with your employer to see if other vaccination plans have already been made. If not, you can be vaccinated at the following places:<\/p>\n<h3>Things To Know Before Scheduling Your Appointment<\/h3>\n<p>You can now schedule appointments at the below vaccination sites.\u00a0 You will need to complete the\u00a0<a class=\"exitlink\" href=\"https:\/\/hca-nys.org\/wp-content\/uploads\/sites\/11\/2020\/12\/doh-covid-vaccine-form-instructions.pdf\">NYS COVID-19 Vaccine Form<\/a>\u00a0(PDF) in order to get vaccinated. NYS requires the provider administering the vaccine to check that you completed the form.<\/p>\n<p>In addition, you will need to bring proof of employment, such as an employee ID card, a letter from your employer or affiliated organization, or a recent pay stub.<\/p>\n<p>Before you visit a site for your vaccination, keep in mind the following tips:<\/p>\n<ul>\n<li>Reschedule your appointment if you are not feeling well on the day of your appointment.<\/li>\n<li>Wear a face covering to your appointment.
